Whenever there is an accident, incident, or even a “near miss”, a thorough investigation will be needed to identify and address its root cause. An investigator, either working alone or as part of a team, will conduct interviews, gather all relevant facts, and provide a detailed description of the incident. They will report the names of all involved parties, specifics as to where and when the incident occurred, and the sequence of events. From there, they will propose a recommended course of action to prevent similar occurrences from happening in the future.
This course teaches the procedures and tools needed to carry out a comprehensive investigation and put forward an appropriate course of action to pre-empt repeat accidents or incidents.
